    A 25-year-old female presents to the outpatient psychiatry office for emetophobia (a fear of vomiting). She reports her symptoms have been going on for “as long as I can remember.” She describes that even thinking about vomiting will make her feel nauseated. She avoids food for days at a time to have an empty stomach to circumvent throwing up. She constantly washes her hands in an effort to avoid germs that could potentially lead to emesis from a gastrointestinal illness. She reports that these symptoms control her life.

A 34-year-old male with a history of hypertension and anxiety presents to the emergency department (ED) accompanied by his wife. He complains of abdominal pain from an injury to the right upper quadrant of his abdomen secondary to a pellet gunshot. He is tachycardic but otherwise hemodynamically stable. The ED physician is suspicious that this may have been a self-inflicted gunshot wound. The man smells of alcohol and does not provide much information about how the injury happened. Routine laboratory tests are ordered, and a psychiatry consult is placed.

    Charlee reviews chapter 5 from the Pediatric Morning Report book. A girl is born at 35 weeks and 3 days gestation to a 26-year-old mother. Minimal prenatal care was given prior to delivery. She is born under considerable distress and is apneic on delivery. The child is noted to be cyanotic, with pulse oximetry measuring oxygen saturation of 50%. Initial resuscitation efforts are focused on ensuring adequate ventilation, but despite 100% inspired oxygen delivered via endotracheal tube, with evidence of gas exchange via carbon dioxide detector, the oxygen saturation remains at approximately 70%.
