In a groundbreaking development, an international team of researchers has unveiled a remarkable advancement in the fight against progressive multiple sclerosis (MS). Their study, led by scientists from the University of Cambridge, the University of Milan Bicocca, and Hospital Casa Sollievo della Sofferenza in Italy, demonstrates the safety, tolerance, and long-lasting effectiveness of injecting a specific type of stem cell directly into the brains of patients living with progressive MS. This breakthrough paves the way for the development of advanced cell therapy treatments for this debilitating disease that affects over 2 million people worldwide.
Multiple sclerosis is a complex autoimmune disease that targets the central nervous system, leading to severe disruption in the transmission of nerve signals. While there are treatments available to alleviate some of the symptoms and reduce the frequency of relapses, many patients still progress into a secondary phase of the disease, which steadily worsens over time, causing severe disability. This secondary progressive phase has long been a significant challenge in the treatment of MS.
In progressive forms of MS, a type of immune cell known as macrophages, specifically microglial cells, plays a crucial role in attacking and damaging the central nervous system. This relentless assault leads to chronic inflammation and extensive nerve cell damage. Recent advances in stem cell therapies have raised hope that they could help mitigate this damage by replacing damaged cells with healthy ones.
Before the current study, the University of Cambridge team had already demonstrated in mouse models that reprogrammed skin cells into brain stem cells, transplanted into the central nervous system, could reduce inflammation and potentially aid in repairing MS-related damage.
This groundbreaking study marks a significant step forward in the pursuit of effective treatments for secondary progressive MS. Although the study is relatively small and may have confounding effects from immunosuppressant drugs, its safety and long-lasting effects over the 12-month trial period have paved the way for further clinical trials. The research team's cautious optimism fuels hope for the development of cell therapy as a potential solution for this debilitating condition. This exciting progress holds the promise of a brighter future for the millions of people living with progressive MS, offering renewed hope for a more effective treatment approach.

With so much discussion these days centered on metabolic health and issues like overweight, obesity, diabetes, and the downstream consequences of these health challenges, I think it's important to spend some time looking at what may be one of the pivotal causes of metabolic disturbances, the consumption of fructose sugar.
As such, I reached out to Dr. Richard Johnson, a worldās authority on the role of fructose in human metabolism, to share with us his latest thoughts not only related to fructose, but also including the various types of sweeteners that are available and have become very popular.
This discussion will take us into the world of GLP-1, a receptor that is stimulated by drugs like Ozempic. And no doubt, that is an area of discussion that is interesting for so many people.
We begin our discussion however with Dr. Johnson describing his recent trip to Dubai where he attended the COP 28 climate conference which, interestingly, includes his insights related to the threats posed by climate change on human metabolism.
Let me tell you a little bit more about Dr. Richard Johnson.He was formerly the chief of the Renal Division and Hypertension at the University of Colorado for nine years. Heās a physician, trained in internal medicine, infectious disease, and nephrology. Dr. Johnson is a widely cited NIH-funded scientist who has lectured in over 40 countries and has authored three books, The Sugar Fix, The Fat Switch, and the new Nature Wants Us to Be Fat. Rick has a special interest in the role of sugar and especially fructose and its byproduct uric acid in driving metabolic and kidney disorders. And most recently he has been exploring the biological properties of a unique sugar, allulose, a topic we discuss in great depth.

Dr. Johnson has performed medical research since the early 1980s and has been funded by the NIH since 1987. Most of his research has focused on the pathogenesis of kidney diseases, but during the last 12 years his studies have also included studies of the pathogenesis of primary hypertension, insulin resistance and diabetes, diabetic nephropathy, and obesity. Much of his work has involved a combination of cell culture and animal models with a strong translational component, but he has also been involved in a number of clinical trials and epidemiology-based studies.

Clearly, we've communicated a lot of information indicating that it's really fundamentally important for our health and ability to resist disease that we gain control over things like anxiety, lack of restorative sleep, lack of exercise, and certainly make other good choices like eating appropriately as well. Anxiety issues are certainly pervasive, and becoming much more common in our very stressful world. Generally, to go to therapy for dealing with anxiety seems to be pharmacotherapy, in other words, drugs. And while drugs may take the edge off and prove to be effective, it's good to know there are other ideas that are being considered. One important inroad into treating anxiety focuses on dietary change. We know that the consumption of the typical standard American diet is fraught with issues including posing a threat to metabolism by virtue of our consumption of large amount of ultra process foods which actually make up between 52 to 54% of of the calories consumed every day in American adults. This is a diet that has been associated with a dramatic increase risk for metabolic issues, cardiovascular disease, and even threatens the brain. As we will learn today, our consumption of these highly refined foods plays a role in creating anxiety as well.

Without question, melatonin may well be the most popular nutritional supplement that people are talking about these days, and with good reason. There is an extensive body of science that looks at the use of melatonin in humans and the results of the research are certainly intriguing.
Dr. Deanna Minich has been at the forefront of melatonin research for the past several years. She brings to our attention some interesting new insights as a relates to using melatonin, including ways to optimize and personalize its use. Dr. Ray Dorsey is the David M. Levy Professor of Neurology at the University of Rochester. Ray is working to identify and eliminate the root causes of Parkinsonās disease. His research on brain diseases and digital health has been published in leading academic journals and featured in multiple news outlets. In 2020, Ray and his colleagues wrote Ending Parkinsonās Disease, a book that provides a prescription for ending the worldās fastest growing brain disease.
Ray previously directed the movement disorders division and neurology telemedicine at Johns Hopkins and worked as a consultant for McKinsey & Company. In 2015, the White House recognized him as a āChampion for Changeā for Parkinsonās disease.

Dr. Bryan earned his undergraduate Bachelor of Science degree in Biochemistry from the University of Texas at Austin and his doctoral degree from Louisiana State University School of Medicine in Shreveport where he was the recipient of the Deanās Award for Excellence in Research. He pursued his post-doctoral training as a Kirschstein Fellow at Boston University School of Medicine in the Whitaker Cardiovascular Institute. So-called "long Covid" is turning out to be a devastating condition for an incredibly large number of individuals around the world. It's been estimated that as many as 200 million people globally are suffering from this condition. And as many as 30 percent of Covid-19 survivors still experience symptoms long after their acute illness has passed, with cognitive and mental health problems especially prominent.

I'll never forget back in medical school that we were told that the brain was "terminally differentiated.ā This means that brain cells were what they were, and could never be regenerated. As such, when a person had a stroke or brain injury, that was pretty much the end. There would be no improvement.
That understanding stands in stark contrast to what we have observed. Clearly, stroke patient can and do improve. And, it turns out, that this improvement can be enhanced with a type of therapy that applies pure oxygen under pressure to the patient. This increased oxygen in the brain does some wonderful things including, importantly, enhancing the growth of new brain cells. What a concept.
I have a long experience with hyperbaric oxygen therapy, having founded one of the countries largest hyperbaric centers. Much of the data upon which we relied came from researchers in Israel who were and continue to do cutting edge science demonstrating the profound effectiveness of hyperbaric oxygen therapy as it relates to stroke recovery. Nowadays, improvements in brain functionality can actually be visualized with state of the art brain imaging equipment.
Today, on the Empowering Neurologist, I have the great honor of interviewing Dr. Shai Efrati, one of the global leaders in the application of hyperbaric oxygen therapy for the treatment of stroke. Dr. Efrati is Founder and Director of the world-leading Sagol Center for Hyperbaric Medicine and Research at Shamir Medical Center, where he also serves as Director of Research and Development and Head of Nephrology. Dr. Efratiās research focuses on novel aspects of hyperbaric medicine and brain rehabilitation. He is a professor at the Sackler School of Medicine and the Sagol School of Neuroscience in Tel Aviv University. Since 2008, he has served as Chairman of the Israeli Society for Diving and Hyperbaric Medicine.

That various microbes in the digestive system affect our health is certainly not news. However, this discussion is typically in the context of the various microbes that live in the gut. Actually, we technically need to begin this understanding at the level of the mouth. And, unfortunately, the idea that our oral microbes are influencing our health has not really gotten a lot of attention. As it turns out the state of health, diversity and function of the microbes living within the mouth are highly influential in terms of our ability to resist disease and remain healthy. Recently, there's been a lot research relating the oral microbiome to such maladies as coronary artery disease and even Alzheimer's disease itself. This will just one of many fascinating topics in my discussion today with Dr. Mark Burhenne.

As many of you may know, we live in Southwest Florida. And yes, we did experience the devastating effects of the recent hurricane. One of the things that has become an important focus of attention for many people living in this area has been remediating their homes as it relates to the possibility or even the presence of mold.

We've had so many discussions that's far about the importance of dietary fat for health and longevity. But to be clear, the type of fat we consume is of incredible importance. These days our diets seem to be loaded with vegetable oils and seed oils, and as we will learn, these are generally quite threatening to our health. In fact vegetable oils are only recently introduced into human nutrition with global production increasing 1600% since the early 1900s.
There is a strong correlation between the increase consumption of vegetable oils and seed the oils with the incidence of severe chronic diseases including cardiovascular disease, diabetes and even various neurological conditions.
Today we are going to explore a new type of oil called cultured oil, prepared by fermentation and subsequent filtration.
