The Build Your Smile Dental Foundation is based on the principle that dental care is a basic human right and that it should not be a luxury afforded to only a select few. This mission has seen the BYSF team form partnerships around the globe -  locally in Toronto, to Costa Rica, Uganda, and Mexico. What began as providing basic dental care - hygiene and extractions - to underserved and impoverished communities quickly evolved to include innovating creative solutions for the delivery of more complex procedures. These procedures include implant placement and 'Dentures-in-a-Day'.

Its services are not limited to treatment alone. A core objective of its mission is to ultimately make their services obsolete in outreach communities by providing dental education and clinical training to dental professionals in those remote areas so that they can continue to provide ongoing dental care to their communities in our absence and ultimately allow us to move forward and partner with new communities. BYSF brings quality dental care to everyone who needs it. The only limitation is the number of organizational partners who join their mission, and the number of dental and oral care providers who want to give back.

Its work would not be possible without the kindness and dedication of volunteers and the caring generous donors. If you are interested in joining the BYSF team or learning more, please visit buildyoursmile.org

The podcast is a discussion with Dr. Nudera regarding his new textbook: NuEndo ReThinking Endodontics.
Knowledge about the science of endodontics can be separated from its clinical application. It is possible to technically perform root canal treatment without any knowledge of the science whatsoever. It is equally possible to understand the science completely, yet not understand how to execute the technique. Although one can find several titles related to endodontics, there remains a clear need for an approachable and understandable text about how the process is carried out and how the physical evidence correlates with the theoretical and technical framework. This book attempts to do that.

This book is a teaching tool as much as a learning tool. Many techniques described in these pages are neither original nor novel. Most of the ideas and concepts are derived from the volumes of clinical research and textbooks available to all. This book merely presents an interpretation of those ideas and concepts, arranged systematically and logically. This book simplifies a complex process. It is for those who want to understand endodontics more deeply from the perspective of a clinically practicing endodontist.

There are known limitations in the techniques. Our models have a strong congruence with the knowledge of integrated biological systems and their relationship to external variables; that is why the clinical practice of endodontics continues to produce mostly favorable results. However, our current models fall short of satisfying all we think we understand about the science. They do not explain why some treatments appear to succeed even though they clearly violate our accepted model. We know a tremendous amount about how endodontics works, but not nearly enough.

How Greenviu started: "Over two years ago we began our journey to help medicine develop a more sustainable base. Our biggest challenge was to find scientific evidence and use that to calculate models on which to build a standard that we could develop into a comparable, fair, and transparent evaluation and certification model. Is our solution perfect? Or is it at least flawless? Sorry, it is not. It’s imperfect, contains estimations, and has faults. But it’s the best algorithm possible with the available evidence. It is designed to learn and to be improved as we gather more information.

The research that will benefit the broadest audience is published on our library page. Next, we began our search for methods and products that could make medicine more sustainable. This required that we discover which actions within the medical field pose the biggest threats to nature and if our ecological footprint can be improved without tampering with quality and hygiene management. Covid 19, for example, has made medicine’s ecological impact far worse than ever before. The abundant use of protective garments, disinfecting agents, and other measures have reached record heights. Covid 19 showed us that we can no longer wait to begin action towards making medicine green."

  • Pohl S, BInderman I, Tomac J. Maintenance of Alveolar Ridge Dimensions Utilizing an Extracted Tooth Dentin Particulate Autograft and Platelet Rich fibrin: A Retrospective Radiographic Cone-Beam Computed Tomography Study. Materials (Basel) 2020 Mar; 13(5): 1083.

    Abstract

This study utilized radiographic comparative analysis in order to evaluate dimensional ridge changes four months after tooth extraction and immediate grafting with mineralized dentin particulate autograft and chopped platelet-rich fibrin. Fifty-eight extraction sockets with up to 2 mm of missing buccal bone in the coronal aspect compared to the lingual bone were included. Graft material was covered with either a platelet-rich fibrin membrane or collagen sponge with no effort to achieve primary closure. The dimensional changes of the ridge were assessed on cone-beam computed tomography (CBCT) images acquired prior to extraction and four months later. The reduction in the buccal bone plate thickness 1 mm, 3 mm, and 5 mm below the buccal crest was −0.87 ± 0.84 mm, −0.60 ± 0.70 mm, and −0.41 ± 0.55 mm, respectively. The mean ridge width changes 1 mm, 3 mm, and 5 mm below the crest were −1.38 ± 1.24 mm, −0.82 ± 1.13 mm, and −0.43 ± 0.89 mm, respectively. The average mid-buccal bone height gain was +1.1%, while the mid-lingual height gain was 5.6%. A mineralized dentin autograft with platelet-rich fibrin is effective in preserving post-extraction alveolar ridge dimensions.

  • Drs. Pohl and Buljan explain the value of dentin autografts. Using autogenous dentin demineralized graft provides a single-stage treatment plan, i.e., extraction of tooth followed by autogenous demineralized dentin graft in the same extraction socket in adult patients – is an alternative for the immediate reconstruction of alveolar bone defects to facilitate the future prosthesis. It also saves the cost of other graft materials which are commercially available in the market for the patient and also reduces the infective dental waste globally.

Dr. Pohl graduated in Medicine at the School of Medicine, University of Zagreb, Croatia, and obtained her medical doctorate in 1987. In 1997 she graduated from dentistry at Ludwig-Maximilian-University, Munich, Germany, and obtained her specialty in Oral Surgery in 2003 at the same university. She acquired her doctorate in dentistry at the University of Cologne, Germany in 2002, and in the same year, Dr. Pohl obtained her specialization in Periodontology and Implantology from the European Dental Association (EDA).

Dr. Pohl is an assistant professor at the Department of Oral Medicine and Periodontology at the School of Dental Medicine of the University of Rijeka and head of the Department for Oral Surgery.  She is an accomplished researcher and published numerous articles in scientific magazines.

In 2020, she published her app to share knowledge and to help practitioners around the world in this specialized field. She brings to the attention that there are no sponsors or companies behind the app, only her own time, dedication, and passion. The procedures in “Decision Matters” are based on what she has found to deliver the best most predictable results.

An early adopter of Partial Extraction Therapy, she is an advocate for the value in dentin autograft, tuberosity tissue, and osseodensification. In 2003 she started to combine hard and soft tissue augmentation techniques to treat the Socket Type 2 and 3 and the mIVAN technique was born. In 2020, long-term results of up to 11 years with mIVAN technique were published.

Dr. Mia Buljan graduated in 2008 from the Faculty of Dentistry in Zagreb, Croatia subsequent to which she began practising with a focus on the fields of oral surgery, implantology, and periodontology. Her clinical experience motivated her to enroll in an Oral surgery residency. Currently, she is doing her Doctorate in Oral surgery at the University of Zagreb while working in the general hospital of Slavonski Brod. In Dr. Snjezana Pohl, she found a kindred spirit and has worked with Snjezana to train in the latest techniques in oral surgery and implantology such as immediate implant placement, Partial Extraction Therapy, and osseodensification.

  • Influence of single post, oval, and multi-post restorative techniques and amount of residual tooth substance on fracture strength of endodontically treated maxillary premolars. Spiccarielli V., Marruganti, C., Di Matteo C., Martignnoni M., Ounsi H., Doldo, T., Ferrari, M., Grandini, S. J Oral Sci 2021;63.

Purpose: The aim of this study was to compare the influence of the number of coronal walls and post-endodontic restorations on the mechanical strength of 165 recently extracted endodontically treated maxillary premolars.

Methods: The teeth were divided into 3 control (no post) and 3 test groups according to the number of residual walls. Each test group was divided into subgroups according to the type of post-endodontic restoration (single, oval, and multi-post techniques). Samples were prepared conforming to the assigned subgroup. A universal loading machine applied a load parallel to the longitudinal axis of the teeth, thus simulating physiological occlusion. ANOVA and the Kruskal Wallis test were used for comparisons (P ≤ 0.05), and Tukey’s test for multiple comparisons.

Results: For intact teeth, significant differences were found among all 3 subgroups, with single post showing the highest values. For 3 residual walls, oval post resulted in significantly lower values than single and multi-post systems. For 2 residual walls, the multi-post technique showed almost twice the resistance of oval post restorations.

Conclusion: In oval canals the use of a single or multi-post technique increased post-and-core resistance even in intact teeth, whereas oval fiber posts showed no improvements. Multi-post design improved fracture resistance mostly in maxillary premolars lacking both marginal ridges.

  • Many digital instruments, techniques, and concepts can be used for smile design, implant planning, and placement, as well as during the implementation of the restorative part. They are quite widely presented in media, published, and recommended. However, only on rare occasions, they are validated through proper laboratory and clinical research. Moreover, published research evaluates only a single aspect or small portion of the workflow. Also, it is quite often that once published, data about digital instruments is outdated already.

Accuracy of the digital workflow for teeth- and implant-supported restorations depends on a clinical situation, selected scanning and 3D printing devices, use of different strategies as well as selected implant and prosthetic components. Therefore, digital workflow for different clinical cases should be planned in advance and specific strategies have to be employed in order to maximize the treatment outcomes with selected devices.

Accurate Digital Implantology and Prosthodontics (ADIP) workflow for certain clinical scenarios is produced through the research of each clinical and lab step of the digital workflow with carefully selected dental implant systems and components. Main aspects being considered in ADIP workflows:
