Where to begin? It’s been a few weeks since you’ve heard from us – thank you so much for your amazing support and patience. As you may know, Hurricane Helene brought devastation to western North Carolina, eastern Tennessee, South Carolina, Georgia, and Florida. Genova Diagnostics is headquartered in the heart of Asheville, North Carolina. With that, it’s been a challenging few weeks. But the heroic efforts of many, and the resilient spirit of our team have us back up and running. Genova continues to support our local community- many of whom are still struggling and have lost everything. #WNCStrong
Today’s episode is a bit different than usual. It’s our story. Our lessons learned. Genova’s story of resilience when faced with a devastating, historic hurricane.

Mast Cell Activation Syndrome (MCAS) has become a hot topic in functional medicine. Mast cells are an integral part of the body’s immune system. They can be found in areas of exposure such as mucosal membranes, the lining of the GI tract, skin, and airways. Mast cells are responsible for storing important chemical mediators like histamine and others which help to instigate allergic responses. Mast cell disorders arise when there are too many mast cells, or they become hyperreactive. That being said, these conditions are often misunderstood, and diagnoses are sometimes misused.
In today’s episode, we focus on the role of mast cells and histamine in the immune response. We discuss the different mast cell activation disorders, how to diagnose them, and what you can do about it.
Today on The Lab Report:
3:05 Mast Cell Activation Diseases 5:15 What is a mast cell? Why do we care? 7:05 Symptoms of histamine release 8:40 Histamine intolerance vs. MCAS 11:11 Foods: High Histamine or Low Histamine? 14:25 Non-food related histamine associations 15:30 Diagnosing and treating MCAS 18:00 Question of the Day What Genova profiles can help parse out histamine intolerance v. The gut microbiome harbors trillions of commensal organisms which play a central role in our health. This community is sometimes referred to as our hidden metabolic organ due to its immense impact. Our diet and lifestyle play an important role in feeding the microbiome and keeping the colonies in balance. Should we be concerned if any one bacterial colony overgrows?
Today we discuss the commensal bacteria Desulfovibrio. Since this genus of bacteria can reduce sulfate and make hydrogen sulfide gas, overgrowth can have clinical implications. Listen in for more information on why you should care and what you can do about it.
Today on The Lab Report:

There are a lot of terms bantered about in the biohacker and longevity space. With that, you’ve likely heard of mTOR – mechanistic (mammalian) target of rapamycin. But what exactly is it? mTOR is a key regulator in your body’s metabolic and growth processes. It integrates various signals, including nutrients and growth factors, to control processes like protein synthesis, autophagy, and lipid metabolism. Because it regulates growth, dysregulation of mTOR has been implicated in various diseases, including cancer.
In today’s episode, we simplify the science behind mTOR – mechanistic target of rapamycin. This crucial enzyme may be the key to unlocking groundbreaking advancements in wellness, anti-aging, disease prevention, and health optimization.
Today on The Lab Report:
3:00 mTOR – Why are we even covering this topic? 4:40 What is it? Muscle growth vs. autophagy 6:25 Why is it called mTOR? 9:00 Highly conserved and in every cell, every organ 10:30 mTORC1 and mTORC2 11:25 Influences and sensors – it’s more than leucine 14:25 Dysregulation of mTOR – does protein cause cancer? 17:30 Should people take rapamycin for longevity? 20:20 Question of the Day Where does mTOR work in muscle protein synthesis?Additional Resources:

Multi-omics is the layering of genetic information (genomics) with laboratory biomarker results (metabolomics) to understand the root cause of disease and risk. This educational series focuses on genetic variants in specific enzymes and explains how to evaluate the systemic effects.
Today’s episode revolves around the enzyme cystathionine-beta-synthase (CBS). This enzyme is important for homocysteine metabolism, sitting at a branch point between two important cycles: methylation and transsulfuration. The action of this enzyme is important for balancing the critical functions of methylation with the creation of glutathione and energy. Listen to understand why this enzyme is important and what a genetic variant in CBS might mean for you and your patients.
Today on The Lab Report:
2:20 Reminder of what we mean by multi-omics 4:50 Introducing the CBS enzyme and the sink drain metaphor 7:40 The genetics of CBS – the silent mutation – and its potential effects 12:40 Better Know A Biomarker: Cystathionine! 16:20 Activators and inhibitors of CBS and its effects on other biomarkers 18:55 Question of the Day: Does a CBS SNP cause elevated ammonia levels? 21:40 Evaluating surrounding biomarkers and how do you treat this SNP?Additional Resources:

Is it possible to be drunk without drinking alcohol? Actually, yes. There is a condition called auto-brewery syndrome whereby an imbalance in your gut microbiome can ferment carbohydrates to create alcohol. Blood alcohol levels can get surprisingly high. Though this condition is thought to be rare, it might just be underdiagnosed. Patients are dealing with legal ramifications, social stigmas, and disbelief by friends, family, and physicians.
Today we introduce the topic of auto-brewery syndrome to raise awareness that your gut microbiome can produce alcohol. We discuss how it occurs, how to diagnose it, and some strategies to treat it.
Today on The Lab Report:
3:10 What is auto-brewery syndrome? 5:30 Causes and clinical associations 9:45 SIBO/SIFO – should we be checking alcohol levels? 10:50 Chronic tolerance and long-term alcohol effects 12:20 How to diagnose ABS 14:30 Treatment strategies 17:00 Is it as rare as we think? Is there a spectrum? 18:12 Question of the Day What patterns might we see on GI Effects and NutrEval?Additional Resources:

Food labels can be very confusing. In fact, many of us completely ignore them. If you look at the ingredients in many processed foods, you’ve probably tried to pronounce some of the chemical compounds listed in the ingredients. The food industry adds chemicals to food to either enhance flavor and color, or to preserve shelf life. There are thousands of accepted food additives and some may have important health implications.
In today’s episode, we discuss just a few additives you might want to keep an eye out for next time you go to the grocery store.

Dr. Stephanie Venn-Watson dedicated her life to public health and veterinary epidemiology. In studying dolphins, she discovered the first essential fatty acid to be found in over 90 years – C15:0 (pentadecanoic acid). Dr. Stephanie’s discovery and further research on C15:0 has uncovered some amazing cellular health benefits and significant potential for disease prevention. With this knowledge, Dr. Stephanie pivoted and founded fatty15 - a company that produces the world’s first, and only science-backed, patented C15:0 supplement.
In today’s episode, we speak with Dr. Stephanie Venn-Watson about her latest research into the newly discovered essential fatty acid C15:0 and how supplementation with fatty15 could be a game-changer for cellular health.

Methylation is a very important enzymatic reaction responsible for so many crucial metabolic processes. In a previous episode, we introduced the topic of methylation and the basics of this biochemical reaction. However, there are many subtle nuances to consider when assessing whether a patient’s methylation cycle is out of balance.
Today, we dive a bit deeper into the biochemistry of methylation and peel back the layers to show how some of the specific biomarkers and backup pathways contribute to imbalance. And, what does that mean clinically? It may help to review our prior episode or use Genova’s Methylation Pathway Handout to follow along.
Today on The Lab Report:
