Hello Fearless Flyers, we continue our discussion with Adrian about the disastrous effects of when flight crew lose their situational awareness, the good news however was that the regulators had sat up and mandated that new technology be applied to reduce this ongoing issue. And a brilliant avionics engineer Don Bateman and his team would ultimately come to the rescue.

In this episode James and Grant have a guest host Adrian joining them to discuss an accident which was too similar to the previous episode on Air Fance 447. Some operators just didn't want to incorporate the lessons learnt from AF447, however we finish on another incident that had a positive outcome, indeed most operators had taken the lessons on board.
