AUA2026: Focus on: ADT in the Modern Era - Balancing Cancer Control and Cardiovascular Safety

LEARNING OBJECTIVES:
After participating in this CME activity, participants will be able to:
1. Integrate cardiovascular (CV) risk assessment into ADT initiation by applying key findings from the REVELUTION-1 trial to identify patients at elevated risk for atherosclerosis and MACE.
2. Compare and apply the CV safety profiles of LHRH agonists versus GnRH receptor antagonists to guide personalized ADT selection in patients with varying CV risk.
3. Implement streamlined monitoring strategies for men on oral ADT, including testosterone levels, lipid and blood pressure evaluation, and surveillance for early CV risk indicators.
4. Collaborate with cardiology and primary care to optimize statin use and coordinate CV risk-reduction strategies for prostate cancer patients receiving ADT.
5. Strengthen shared decision making and adherence by incorporating discussions of oncologic benefit, CV risk, and mitigation strategies into routine oral ADT counseling and follow-up.

1. Critically appraise recent developments in imaging biomarkers and molecular diagnostics (e.g., genomic risk scores, liquid biopsy) and discuss how they complement MRI and PSMA PET in prostate cancer care.
2. Determine the optimal clinical scenarios (diagnostic, staging, recurrence, surveillance) in which PSMA PET/CT or PET/MRI adds value beyond conventional imaging.
3. Recognize potential pitfalls in PSMA PET interpretation — including false positives, false negatives, non–PSMA-expressing disease, and technical artifacts — and implement strategies to mitigate them.
4. Synthesize biomarker, MRI, and PSMA PET findings into a personalized management plan, including guiding biopsy, focal therapy, salvage therapy, or systemic treatments.
5. Select appropriate patients with radiorecurrent or organ-confined prostate cancer for advanced imaging and guide re-treatment decisions (e.g., salvage therapy, targeted radiotherapy).

AUA2026 Spotlight: PARP-Inhibitor Combination Treatments for the Urologic Care Team

At the conclusion of this CME activity, participants will be able to:

Integrate biomarker and genetic testing principles into clinical workflows for patients with metastatic prostate cancer, including when to order testing, how to interpret HRR mutation results (inclusive of BRCA and non-BRCA), and how to address barriers to testing through multidisciplinary coordination.
2. Explain the mechanism of action of PARP inhibitors and the biological and clinical rationale for their use—both as monotherapy and in combination approaches—in the treatment of mPC.
3. Evaluate emerging efficacy and safety data on PARPi combinations, including patient subgroup analyses, sequencing strategies, and the role of combination therapy in different stages of mPC.
4. Apply best practices for side effect monitoring and mitigation in patients receiving PARP inhibitors alone or in combination, leveraging the multidisciplinary team for optimal therapy management and patient quality of life.
5. Implement guideline-concordant care strategies in practice, including genetic testing workflow implementation, coordination among care team members, and patient engagement in shared decision-making and clinical trial enrollment.
5. Utilize current evidence-based guidelines to select and sequence PARP inhibitor therapy for patients with mPC, optimizing oncologic outcomes while individualizing care based on molecular profile and patient-specific factors.
