I’m so excited to interview Dr. Blake Evans on the Egg Whisperer Show podcast today to share his published findings about testicular sperm and IVF outcomes. Testicular sperm (or sperm taken from the testicle as opposed to ejaculated sperm) in some cases may improve the rate of blastocyst development and pregnancy rates for those who have first tried IVF unsuccessfully with ejaculated sperm.

  • I’m honored to be joined by Dr. Carol Curchoe, who is a reproductive biologist practicing clinical embryology. Her active research interests span reproductive biology, from basic embryology to cutting edge artificial intelligence applications for IVF. She is the founder of ARTCompass, which is a free open source image based embryology survey tool, and she is the recent author of The Thin Pink Line: Regulating Reproduction.

    Her book is truly all about empowering people by educating them about present-day drivers of access, structural inequality, and quality of care issues in reproduction. I am glad that Dr. Burton is bringing them to light so we can address them and change things. No discussion of infertility would be complete without this discussion.

  • Dr. Geoffrey Sher is back, and we are going to be covering a very important topic: why do you need to do PGT testing if you’re doing IVF (or do you?) Dr. Sher is the co-founder of Sher Fertility Solutions (SFS) and internationally renowned expert in the field of ART (Assisted Reproductive Technology) and has been influential in the births of over 17,000 IVF babies.

    Around 2005, Dr. Sher's team published the first paper that showed beyond a shadow of a doubt that if you were able to fully karyotype the embryo cells that a euploid embryo would have an excellent chance.

    Today there is hardly an IVF program in the world that is not doing PGS/PGT-A to assess embryo viability. Today, we are discussing if PGT is needed for all IVF cycles, the role DOR plays in assessing if PGT is useful, and other maternal chromosomal and genetic abnormalities that may mean you'd want to consider PGT. Plus, he's addressing the concern that some patients have around the possibility that PGT may harm an embryo (short answer: if done correctly it does not).

  • Dr. Geoffrey Sher is joining me again, and this time we are going to talk about implantation failure during IVF. Dr. Sher is co-founder of Sher Fertility Solutions (SFS) and an internationally renowned expert in the field of assisted reproductive technology, and influential in the births of more than 17,000 IVF babies. One day I hope that I’ll be able to say the same thing about myself. Over the last 30 years, he has helped fashion the entire field of ART after training under the fathers of IVF, Dr. Patrick Steptoe and Robert Edwards. He established the first private IVF program in the United States in 1982, then later expanded his practice to include several centers throughout California before founding the first Sher Fertility Solutions in Las Vegas. He’s also the author of several books, In Vitro Fertilization: The ART of Making Babies, which is now in its third edition.

    In this episode, we're talking about implantation failure during IVF. Many times I wonder why embryos do not stick and grow. Dr. Sher is discussing the role of the endometrium, the difference between implantation and miscarriage, and the three factors he considers when advising on treatment for implantation failure.   • Today, I'm delighted to be joined on The Egg Whisperer Show by Marc Sherman of Organic Conceptions, a company he founded after a 20-year career in corporate America. The idea came after he and his wife Erin struggled with infertility, decided to adopt, and then unexpectedly found themselves pregnant.   • Dr. Camran Nezhat is world renowned for his work in minimally invasive and robotic surgery, and he specializes in treating people with endometriosis.   • Whether you’re an egg donor now, if you’re considering it, or you know someone that is — I’m glad you’re here.

    Young women are donating eggs at the time they are most fertile, often times not realizing the implications of what that means. I’ve had patients come to me in their 40s without enough healthy eggs of their own to conceive. When they share with me that they themselves donated their young and healthy eggs in their 20s or 30s, but that none are available for them, I feel frustrated and sad for them.

    Frustrated enough that I’m doing something about it. Freeze & Share is a program that helps young egg donors save some eggs for themselves should they need them, while helping another family conceive when they can’t on their own.

    Suffice to say, I don’t want anyone to look back at their egg donation and wish they’d known more.

  • This episode is all about how to survive your HSG. Otherwise known as a hysterosalpingogram. That’s a big scary word so let’s break it down!

    Hystero = uterus
    Salpingo = tube
    Gram = picture of

    Hysterosalpingogram (HSG) is basically a picture of your fallopian tubes. I prefer a gentle tube test with Exemfoam.

    I like to think of the fallopian tube as the embryo transport system. It’s where the egg and sperm come together and it’s how the embryo will travel to the uterus. Part of fertility screening is not just for FSH, estradiol, and AMH for women. It’s not just to see how fast the swimmers are swimming. It’s also important to make sure that the fallopian tubes are open! It’s the “T” in the tushymethod.com

    "Is it possible to become emotionally resilient, so that you can handle all of the things that you face as a fertility patient without feeling overwhelmed and triggered constantly?" I get this question all. the. time, and today I'm excited to have Abbe Feder of Incircle Fertility on to share tips, tricks, and mindset shifts with us!

    Tip 1: Be kind to yourself, and know that things like baby announcements, or nosy questions from family members are triggering to many fertility patients. That doesn't make it easier, but it might help to know you're not alone in the feelings. Some of these triggers can come from unexpected places.

    Tip 2: To work through triggers: talk about the trigger, acknowledge the trigger, become aware of what triggers you, and then "figure out what works best for you to get past the trigger" when they do come up.

    Tip 3: Allow yourself to feel your emotions. Don't push them aside. Find a trusted friend, family member, spouse, partner or coach to help you. They don't have to understand what you're going through, the key is that they are able to listen to you and let you talk.

    Abbe Feder is a fertility coach and the founder and CEO of InCircle Fertility. She and her husband host the podcast Maculate Conception, and she is the creator of the (In)Fertility Cards of Affirmation.

    We had the pleasure of meeting at a panel with Fertility Rally and I could not be more excited to have her on the show to talk about how to be emotionally resilient during fertility treatment. Today, we're talking about what it means to be a fertility coach, how to find support for your journey, and how to develop more resiliency when facing the ups and downs of being a fertility patient.
