For some individuals, the presence of implants may trigger a range of symptoms known as breast implant illness (BII), including fatigue, joint pain, and cognitive issues. These complications can occur as the body responds to foreign materials, potentially exacerbating pre-existing health conditions or causing new ones.The impact of breast implants on overall health can be significant. That is why understanding the potential risks is important for those considering breast implants or experiencing symptoms that may be related to them. This episode with our patient, Allie Janszen, focuses on breast implant illness and her removal journey as she shares her personal experiences with the health challenges related to breast implants and how their implant removal led to significant improvements in her well-being. Breast Implant Illness and Weight Loss Struggles after ChildbirthWhen Allie first considered breast augmentation, she was very lean, having been a college athlete. Although she might have had some fat that could have been used for a fat transfer, it wasn’t something she had ever heard of or considered. Her mindset was focused solely on getting breast implants.Fast-forward to after the birth of her fourth child, a difficult emergency C-section triggered what she later realized were symptoms of breast implant illness. Despite her background as a labor and delivery nurse and a fat loss coach – where she was helping others with weight loss and healthy living – she struggled to lose weight and manage her own health. She realized that the same techniques that worked for her clients weren’t working for her anymore.In addition to the weight loss struggles, Allie experienced extreme fatigue, making it hard to wake up in the morning, hitting an afternoon energy wall, and even falling asleep while putting her kids to bed. These symptoms, combined with inflammation and hormonal changes, were all part of what she now recognizes as breast implant illness.Tinnitus, Meniere's Disease, and Inflammation after Breast Implant RemovalDr. Whitfield explains that Tinnitus, Meniere's disease, and sensitivity to light and sound are often related to neural inflammation. While many patients experience improvement or even elimination of these symptoms after implant removal, pre-existing conditions like chronic migraines or autoimmune disorders may not be fully resolved by this procedure. For patients with a higher BMI needing breast reconstruction, alternative methods like the DIEP flap surgery are available. After Allie had her breast implants removed, she noticed significant improvements in her health, particularly in areas she hadn't expected. Prior to the removal, she struggled with joint pain during workouts, experiencing discomfort in her knees and wrists that she had never dealt with before. Additionally, she was diagnosed with Meniere's disease after experiencing persistent symptoms in her left ear, such as a feeling of fullness, vibrating noises in loud environments, and episodes of vertigo. These symptoms were occurring weekly, yet after the implant removal, they completely subsided, which she found to be the biggest change.She had initially been focused on resolving her weight loss struggles and reducing inflammation, but the improvements in her Meniere's symptoms were an unexpected benefit. Despite her background as a health professional, she hadn’t anticipated that removing her implants would have such a profound effect on her ear issues. This experience led her to recognize the connection between her implants and the inflammation that was likely contributing to her Meniere's disease symptoms. The immediate relief she experienced was a clear indication that the implants had been exacerbating her condition.Post-surgery Fatigue and Inflammation Reduction, with a Focus on Diet and Lifestyle Changes for Improved Sleep and Overall HealthAllie’s commitment to a clean lifestyle played a crucial role in her recovery and health improvements. She and her husband had already been avoiding plastics, using natural household products, and maintaining a nutritious diet, which likely contributed to her ability to bounce back quickly after the surgery. Her weight loss and other health improvements can be attributed to the reduction of inflammation caused by the breast implants, which she believes had been disrupting her hormones and overall well-being. In addition to the weight loss, she noticed a drastic improvement in her energy levels and the elimination of afternoon fatigue. This boost in energy and the resolution of her fatigue suggests that her body has undergone a significant reset, particularly in her mitochondrial function, which has improved her overall metabolic efficiency.Dr. Whitfield praised Allie’s high protein intake for reducing fluid retention and aiding recovery, which improved her sleep and energy levels. He highlighted the importance of a consistent sleep schedule, reducing screen time before bed, and a cool, dark sleep environment for better rest. At the time, she did not consider potential health impacts or the need for future replacements, as these were not part of the discussion.There was no pushback or significant questioning from Kacey's surroundings, as breast augmentation was seen as a routine choice. She mentioned that during her initial consultations, other options such as fat transfer were not discussed, possibly due to the standard practices of that era and the availability of such procedures 20 years ago.Young adults are often resolute in their decisions regarding breast augmentation, which makes it particularly challenging to change their minds once they have made a choice. This is often rooted in a combination of personal conviction, cultural influences, and a strong desire to achieve a specific body image. For many, the decision to undergo cosmetic surgery is influenced by deeply ingrained perceptions of beauty and self-esteem that have been shaped over time by societal standards, media portrayals, and personal experiences.Kasey’s journey also included a transition from saline implants, which she initially received in 2005, to silicone implants in 2010. She experienced complications with her saline implant shifting out of position and sought correction from a different surgeon. In reconstructive surgery, especially for cancer patients, complications frequently arise due to factors like chemotherapy, radiation, and infections, which can affect healing and cause issues such as implant malposition. Even with surgical interventions to correct these problems, implants often shift because of the patient's daily activities and movement. In Kasey’s case, her silicone implants, which were meant to resolve earlier issues, ended up facing the same problems, as they were unable to stay properly positioned due to her active lifestyle.Postpartum Symptoms and their Physical Manifestations, Including Brain Fog, Nerve Pain, and Scalp PainWhen the immune system detects implants as foreign materials, it can initiate an inflammatory response as it attempts to protect the body from these perceived threats. This inflammation can manifest in various ways, including symptoms such as brain fog, nerve pain, and scalp pain.Kasey experienced brain fog, characterized by memory issues and confusion, along with nerve pain in her neck and back, which caused headaches and scalp discomfort. However, she attributed these symptoms to postpartum fatigue and stress. She had planned to have her implants removed but chose to wait until her children were older. Over time, her symptoms persisted and did not improve despite various remedies, leading her to suspect a connection to her breast implants rather than just postpartum changes. Despite trying cold therapy and other lifestyle changes, her condition only improved after her implants were removed.Risks of Sauna Use with Breast ImplantsDuring Kasey's consultation, I advised her to avoid using saunas, particularly those with high temperatures. This recommendation was based on observations from other patients, where exposure to extreme heat from saunas appeared to increase heavy metal toxicity from degraded implants, leading to worsened symptoms. Despite the lack of concrete evidence on how high temperatures impact implants, the cautious approach was to prevent potential health risks associated with sauna use.Kasey found the advice to avoid saunas sensible, particularly considering her own genetic predispositions that affect detoxification. Her experience, combined with observations from other patients who had similar issues with high-temperature environments, reinforced the importance of steering clear of saunas until more is understood about their impact on health.Detoxification and Its Impact on Health, Including a Patient's Experience with the CellCore Detox ProgramFunctional genomics is a big change, significantly enhancing our understanding since the completion of the Human Genome Project. These advancements have enhanced our understanding of genetic pathways related to detoxification, such as vitamin D metabolism, methylation, and antioxidant functions, as well as hormone metabolism, mood, and sleep.Kasey's situation illustrates how her genetic limitations in detoxification have impacted her recovery process. Despite her efforts and the use of detox programs like CellCore, she initially had a tough time. Recent tests showed increased levels of toxins, which might suggest that her liver is becoming more effective at removing these substances from her body.Guided by a detox expert, she has been following a tailored detox plan and specific supplementation regimen. She has been feeling generally well, with good energy levels despite some fatigue during the detox process. The ongoing detox efforts and tailored supplementation are helping improve her overall condition and liver function.As Kasey nears the end of her detox program, ongoing work will be necessary to address her condition. There is no single test that can definitively diagnose breast implant illness, but advancements are being made in this area. Researchers are exploring new methods to better understand inflammation levels and related symptoms.Current testing methods like sed rate and CRP tests are insufficient for accurately diagnosing breast implant-related issues. With ongoing studies and research into bacterial contaminants like staphylococcus epidermidis and biofilms, more accurate diagnostic tools are being developed to improve patient care and treatment outcomes.Breast Reconstruction, Mental Preparation, and SupportFor future patients considering breast reconstruction, it’s crucial to understand that recovery takes time, both physically and mentally. While many people focus on the cosmetic outcome, it’s essential to allow the body to heal fully, including detoxifying and adjusting to new physical changes. The process of reversing cosmetic procedures can be challenging, and achieving a baseline look and feel may be difficult. In Kasey's case, I recommended a fat transfer to improve her appearance due to her low BMI, even though she was initially hesitant. This approach aimed to help her achieve a more natural look and feel.For many patients, including Kasey, the decision to undergo breast augmentation or reconstruction involves more than just physical changes; it also affects their mindset. As people age, gain weight, or experience hormonal changes, their physical appearance can shift, making it important to address these changes with procedures like fat transfers to restore volume. However, a significant aspect that is often overlooked is the mental adjustment required. Patients may face challenges such as trauma, guilt, or shame when dealing with the reversal of cosmetic procedures, and these emotional factors can impact their overall well-being. To support patients better, there is a need for programs that offer more comprehensive mindset support, acknowledging the psychological effects that accompany physical changes.Remember that mental mindset plays a crucial role in the perception and experience of symptoms related to breast implant illness. A negative mindset can exacerbate symptoms like fatigue, pain, and nerve issues, similar to how certain therapies can trigger responses in the body. Support systems are indeed crucial for recovery, and Kasey’s supportive partner plays a key role in her healing process.     Preparation for breast implant surgery, including air quality, water quality, and food quality.
    I often get asked how to start preparing to lower inflammation. Of course, having a genuine support system greatly helps. Surrounding yourself with supportive family members, friends, or loved ones can provide emotional encouragement and practical assistance, making the process more manageable. This support can be especially crucial when implementing lifestyle changes, ensuring you stay motivated and on track toward achieving your health goals.
    Focusing on air quality is vital. Personally, this is important to me considering the fact that I had allergies growing up. I highly recommend Jaspr Air Purifiers because they automatically adjust to air quality, which is unique in the market.
    Another important factor is water quality because filtered water is key to reducing inflammation as it promotes better gut health. That’s why we use Echo filters in our office and surgery center because they do the trick and even reduce oxidative stress.     When it comes to food quality, prioritizing organic vegetables is crucial. Organic vegetables are grown without synthetic pesticides, herbicides, and fertilizers, meaning they are less likely to contain harmful chemicals like glyphosate. Glyphosate exposure can disrupt hormonal balance, particularly in women, and has been linked to various health issues. By choosing organic, you significantly reduce your risk of ingesting these potentially harmful substances.
    Furthermore, organic farming practices enhance soil health and biodiversity, leading to more nutrient-dense produce. Organic vegetables often contain higher levels of essential vitamins, minerals, and antioxidants, which are vital for combating inflammation and supporting overall health. Consuming nutrient-rich foods helps your body maintain optimal functioning and resilience against diseases.
    I highly suggest checking the Environmental Working Group's Dirty Dozen list for guidance on which produce to buy organic and always wash your fruits and vegetables thoroughly. I also recommend cutting out gluten, minimizing dairy, and avoiding seed oils, as these can be pro-inflammatory.
    Focusing on organic vegetables is a simple yet impactful step towards reducing inflammation and supporting overall health.
    Before starting any procedure with us, it's essential to prioritize improving your recovery first. Surgery can trigger inflammation, so managing stress and hormone levels is crucial. Follow the 321321 rule before bed: finish your last meal three hours before bedtime, stop drinking fluids two hours before, and urinate before bed to avoid disturbances. Quality sleep is also vital as it helps in hormone secretion and recovery. In line with this, if you snore, it's best to get tested for sleep apnea, as it can lead to significant health issues.
    By improving your air, water, and food quality, and following these preparation tips, you'll be well on your way to reducing inflammation and improving your overall health.

  • There are many reasons why women undergo breast explant surgery. Although some women do breast explant because they don’t want them anymore, some opt for breast explant surgery due to health issues or complications such as capsular contracture (scar tissue tightening around the implant), rupture, infection, or chronic pain.
    Knowing the expected recovery time gives people reasonable expectations regarding their healing process. It prepares them for the temporary limits and pain they may encounter during this period, which can help lessen worries. In today’s episode, we’ll be talking about how long breast explant surgery and recovery take.
    Optimizing your immune system
    When you undergo an operation, whether breast explant surgery, breast implant, breast reconstruction, or others, you have to ensure that your immune system is in its optimum state.
    To optimize your immune pathways, we do genetic testing and saliva tests which normally take a couple of months to get the results. We also conduct urine tests to identify toxicities and inflammation.
    Another thing that should be taken into consideration is the environment, as it also affects one’s immune system and recovery after a breast explant surgery. If you’re in an environment with molds, you have to be careful as these affect the air quality, which can also negatively impact your health.
    Optimizing your diet and taking supplements
    Having a healthy lifestyle boosts the immune system. Diet obviously plays a key role in all of these because if you can't eat and absorb nutrients, you can't recover easily.
    You have to stay on track with your diet as it limits fluid accumulation by changing your oncotic pressure, which is the protein content in your blood so that you go from a leaky phase or catabolic phase after surgery to an anabolic phase or a non-leaky phase. This helps your swelling to lessen quicker.
    Optimizing your diet includes an examination of food sensitivities and assessing your body’s food and nutrient absorption. We also have to check if you have bacterial, fungal, or parasitic overgrowth or leaky gut. It’s also important to ensure proper blood work and hormone balance for recovery since women have high levels of cortisol and sometimes estrogen as well, which can prolong recovery.
    We carefully look into all of these factors, particularly the levels of estrogen, progesterone, and even testosterone, and optimize them through our supplements to shorten the patient’s recovery time. We also use red light therapy and lymphatic massage. Many of my clients also like to use hyperbaric oxygen therapy.
    Generally, it takes one to two weeks of recovery if you’ve had an explant and it’s above the muscle and a couple of weeks - depending on how hard it is to get it out from behind the muscle. However, the recovery period still depends on many factors like your genetics, diet, lifestyle, environment, and hormone balance.
    If we carefully optimize these, we’ll be able to give you the possible results and decrease the time for recovery. We're also introducing new things to enhance recovery like peptide therapy because we want you to recover as efficiently as possible.

  • Breast augmentation or breast implant surgery has become a more popular surgical procedure throughout the years for various reasons. While it has many benefits, it also comes with potential risks such as capsular contracture, infection, breast implant illness, and more.
    These risks are some of the reasons why many women opt to undergo breast explant surgery. That’s why it’s important to have a qualified plastic surgeon when considering surgery and to carefully assess both the risks and benefits of having breast implant surgery and other procedures.
    What Are the Reasons for Having Breast Explant Surgery?
    My main take from the thousands of operations I’ve done is that we don't want to leave biofilm of any quantity in somebody. So what we do is remove all of what’s intact, if possible.
    Women seek breast explant surgery for many reasons. There are those who want to lessen the pain from symptoms associated with breast implant illness like brain fog, sound and light sensitivity, shortness of breath, heart palpitations or chest tightness, neurologic symptoms in the arms, nerve pain, muscle pain, and joint pain, while some even experience gastrointestinal symptoms.
    Whereas some are just tired and unhappy with their implants, some undergo breast explant surgery for breast reconstruction because they have issues with the appearance of their breasts. Maybe they’re uncomfortable with the size or maybe there are gaps or capsular contracture.
    It’s important to carefully consider having breast explant surgery and to have a qualified plastic surgeon whom you can trust. It also helps to have a great support system around you throughout the process.

    Fat transfer surgery for breast augmentation and reconstruction
    I’ve recently attended several notable conferences where I gained valuable insights and innovations that I’m eager to integrate into our clinic. Some of these advancements include the incorporation of more lymphatic massage techniques, the introduction of NanoVi technology to improve protein folding and recovery, and the use of H-Wave therapy to further enhance lymphatic drainage and aid in patient recovery.
    For today, however, let’s focus on fat transfers. Drawing from my extensive experience with fat transfers, a technique I’ve refined over many years, I explain that fat transfers are effective because they use the patient’s own tissue, making them autologous and thus free from the risks associated with donor tissues, such as genetic mismatches.
    Fat transfers are versatile, as adipose-derived stem cells can differentiate into fat cells, bone cells, or cartilage, providing a range of benefits. My initial interest in this technique stemmed from a personal desire to regenerate cartilage in my knees after multiple surgeries, leading to a deep understanding of the procedure’s potential.
    With extensive experience in sarcoma reconstruction, I can confidently say that fat transfers are safe when performed correctly. It's essential not to transfer fat to areas with existing tumors or sarcomas, as these mesenchymal regions could complicate the procedure.
    For breast augmentation and reconstruction, fat transfers have been effective for many years, and I have conducted numerous procedures since 2005. Successful outcomes depend on transferring a mixture of adipocytes, vascular cells, and adipose-derived stem cells. This technique requires precise placement of fat in specific areas to avoid complications and ensure proper integration with the surrounding tissue.
    There were early safety concerns regarding fat transfers for cosmetic purposes, such as buttock augmentation, due to technical issues when the procedure first gained popularity in South America and Miami. However, with advancements in techniques and improved safety measures, fat transfers for both buttock and breast augmentation have become reliable and effective procedures.
    Fat transfer for breast implant illness, including stem cells and healing
    When the skin gets really tense, you'll notice a condition called "peau d'orange," where the surface of the skin starts to look like the surface of an orange. This indicates too much pressure and that expansion should stop. Unlike implants, which can hold their shape and maintain more volume, fat cannot be continuously added. The amount of fat that can be added depends on the initial condition of the skin. For younger clients with tight skin, multiple sessions may be needed to gradually increase volume.
    In cases where I remove implants that have been in place for a long time, often after weight changes, pregnancies, or breastfeeding, the skin is usually stretched out. After removing the implant with its capsular material intact, I don't fill the old implant pocket and instead, place the fat beneath the skin, where it belongs, and let it heal.
    Adipose-derived stem cells in the fat offer numerous benefits, aiding in wound rejuvenation and healing. They can help turn genes on, produce exosomes, modulate immune responses, and improve mitochondrial function. This is why fat transfers are more than just about the fat itself – they bring substantial healing properties.

  • Breast explant surgery or breast implant removal is a surgical procedure commonly done for various reasons such as implant issues, breast reconstruction, implant rupture, dissatisfaction with the appearance of the implants, or other cosmetic purposes.
    In breast explant surgery, the implants alone may be removed or the surrounding scar tissue may also be included. In some circumstances, a breast lift may be done with the explant procedure to enhance the look of the breasts.
    In today’s episode, we’re going to discuss what breast explant surgery is and why people want to undergo such surgery.
    What is breast explant surgery?
    When we’re talking about breast explant surgery, we’re referring to the removal of breast implants and any scar tissue formed around the implants. Nowadays, many plastic surgeons do explants for various reasons.
    My approach to explant surgery is to everything out in its entirety, which is why many people seek me out for that very reason. Typically, my clients are those who had a placement of a device either for cancer reconstruction or other cosmetic purposes and are experiencing symptoms associated with breast implant illness.
    The most common symptoms experienced by my patients are anxiety, depression, brain fog, dry eyes, headache, light sensitivity, sound sensitivity, shortness of breath, heart palpitations, nerve pain down their arms or legs, joint pain, and other gastrointestinal issues.
    Oftentimes, patients find me through what I offer in terms of holistic rejuvenation with fat transfer. In fact, I’ve been doing fat transfer since the 2000s for cancer reconstruction and other cosmetic purposes. I'm very comfortable with doing these operations and I have already refined my process, having done over 1,500 fat transfers.
    Why do I prefer fat transfer?
    When it comes to breast lifts, we have different kinds of techniques such as vertical, crescent, peri-areolar, and anchor breast lifts. The patients may have a preferred kind but it actually depends on the necessity.
    If the patient had a peri-areolar placement, then we try to use that scar and remove the implant intact through it. The average length of a surgery is about two and a half hours. If we add a lifting procedure, it's a little over three hours.
    If I'm doing a combination procedure of fat removal to be used for fat transfer, it usually takes between four to five hours. The main reason as to why I like to use fat is because it's your own tissue. It has actually been used as a filler for over 100 years.
    Besides providing or enhancing the breast volume, it also helps offset some visual changes that the patient's not happy with. This is why as much as I can, I try to help patients learn about fat transfers and make them understand from my perspective.
    Doing these operations can be draining, especially mentally, so it’s best for patients to have a great support system that shows up to help them and provide much-needed support.

  • Breast implants are said to be a transformative solution for individuals seeking aesthetic enhancement or breast reconstruction. However, beneath their promise of confidence and beauty lie potential risks that are often overlooked. Breast Implant Illness (BII) underscores the importance of thorough patient education and informed consent, highlighting the complexities and implications associated with implants. That is why there is a need for us to raise more awareness about the realities of breast implant surgery, ensuring that patients are fully informed about the risks and potential consequences.
    I'm thrilled to introduce our guest for today's episode – Dr. Thomas Chung. We share a similar background in terms of experience and training, which initially intrigued me since he has a clear and insightful understanding of breast implant illness, and we'll delve into that throughout this episode.
    Plastic surgery career, including microsurgery and breast reconstruction
    What struck me about Dr. Chung is his extensive experience in microsurgery, a field in which we've both worked. Dr. Chung's journey began with his training in the Navy, where he completed his internship and general surgery residency at Walter Reed, then part of the National Naval Medical Center consortium in DC. During his residency, he managed numerous casualties from conflicts like Fallujah, serving as chief of the trauma service with a daily census of 30 to 40 injured veterans. After finishing his general surgery training, he pursued a fellowship at Johns Hopkins, launching his plastic surgery career before returning to Walter Reed to handle more trauma cases.
    His work involved extensive microsurgery, such as reconstructing an entire abdominal wall for a survivor of a suicide bombing and performing numerous extremity-free flaps and breast reconstructions using deep inferior epigastric perforator (DIEP) flaps. Over the first seven to eight years, Dr. Chung climbed the Navy's ranks, taking on leadership roles like Chief of Surgery, Chair of the Medical Executive Committee, and ultimately Chief Medical Officer for the San Diego region.
    Dr. Chung remains passionate about plastic and aesthetic surgery despite his executive responsibilities. Notably, he performs many explant surgeries for patients experiencing issues after long-term breast implant use.
    Immunosuppression in organ transplants and chronic inflammation
    In my previous episodes, I've extensively discussed breast implant illness and Dr. Chung agrees that implants may cause more than just localized pain or discomfort. He believes there's something inherently toxic about implants that science can explain and this understanding aligns with groundbreaking surgical advancements by the Nobel Prize winner, Dr. Joseph Murray. Dr. Murray's work on organ transplants demonstrates the need for immunosuppression to prevent rejection.
    In transplants, immunosuppression is crucial to avoid rejection, but this concept can also apply to implants. Patients often react negatively to treatments like steroids or methotrexate, fearing the medications rather than understanding their role in managing symptoms.
    We see chronic inflammation in patients with implants, similar to organ rejection. In both scenarios, if the initial post-surgery period goes well, complications are less likely. However, issues during this time can lead to significant problems. For cancer patients undergoing reconstruction, immunosuppression can exacerbate complications, demonstrating the delicate balance required to manage immune responses.
    Transplant rejection and the use of splint analogy to explain the concept
    Using analogies from organ transplants helps patients understand that their bodies might be "rejecting" implants, causing chronic inflammation. This perspective can help them grasp the need for proper management and treatment strategies, even if those include higher doses of immunosuppressants like prednisone to manage their symptoms effectively.
    When it comes to resolving issues with breast implants, focusing on the concept of explants and the importance of addressing capsules surrounding the implants is vital. We use the splinter analogy to explain the body's reaction to foreign objects. Just as a splinter in your finger causes pain and inflammation until it's removed, so does the body react to implants, leading to discomfort and potential toxicity. Removing the splinter provides immediate relief, compared to the relief patients might experience after explant surgery.
    Dr. Chung appreciates the simplicity of this analogy, which helps patients understand the body's rejection response. He relates it to his experience on transplant rotations, where managing immune responses to transplanted organs was crucial. He emphasizes the importance of detailed procedures in removing implants, akin to meticulous methods used in cancer surgeries to reduce recurrence risks.
    Breast implant illness, implant removal, and fat transfers as an alternative
    Both Dr. Chung and I have extensive experience dealing with complications from implants, whether in cancer patients or veterans needing reconstructive surgery. We emphasize that implants can be a necessary part of treatment but also come with risks that must be clearly communicated to patients.
    Dr. Chung admitted that he initially didn’t know much about implant-related issues despite performing many breast augmentations. He often dealt with patients who had capsular contracture, a condition where the scar tissue around the implant tightens and causes pain or deformity. In these cases, he would remove the implant and the capsule and then insert a new implant, only for the problem to eventually recur.
    I would like to highlight fat transfers as a viable alternative to implants. Fat transfers can help refine and improve reconstructive results by adding volume, achieving a natural look without the risks associated with implants. Dr. Chung supports this approach, noting that while fat transfers don’t create the same pressure as implants, they offer a safe and effective option for many patients.
    Fat transfer vs implant for breast augmentation, with pros and cons for each method
    Implants provide more projection and pressure, pushing out the skin envelope, especially beneficial for patients with tighter skin or certain deformities like tuberous breasts. Fat transfers, on the other hand, follow the natural curvature of the breast, adding volume and creating a natural look without the risks associated with implants.
    Fat transfers are particularly effective for patients who have more lax skin, often due to previous pregnancies, breastfeeding, or prior implants, which act like tissue expanders. While fat transfers don’t provide the same upper pole fullness or projection as implants, they can achieve a satisfactory result by filling out the skin and providing a natural appearance. The success rate of fat transfers is generally high, especially in younger patients with adequate fat, countering any misconceptions that they don't work well. This method is also frequently used in oncologic reconstruction to refine and improve outcomes.
    The success of fat transfers is influenced by various factors, each playing a significant role in determining the outcome of the procedure. One crucial factor is the age of the patient. Younger individuals, typically those in their 20s, often experience better outcomes from fat transfers. This is attributed to their more favorable hormone levels and metabolism, which contribute to enhanced tissue regeneration and integration of the transferred fat. Additionally, younger patients tend to have healthier skin with better elasticity, allowing for more seamless integration of the injected fat and resulting in a more natural appearance.
    Conversely, older patients, particularly those over the age of 35, may encounter challenges that can affect the success of fat transfers. Hormonal imbalances, a common occurrence with age, can impact the body's ability to heal and regenerate tissue effectively. Metabolic issues, such as slower metabolism or insulin resistance, can also hinder the body's ability to process and integrate the transferred fat. Furthermore, poor diet and nutritional deficiencies prevalent in some older individuals can compromise tissue health and overall healing capacity, leading to suboptimal outcomes from fat transfer procedures.
    On the technical aspects of fat transfer, there is about 20-30% of the injected fat that typically gets absorbed by the body. This process is influenced by the patient's skin condition, with factors like previous pregnancies, weight changes, and age affecting skin expansion and the overall success of the procedure.
    It's essential for patients and surgeons to recognize these factors and consider them when planning and performing fat transfer procedures. Tailoring the approach to address specific challenges associated with age can help optimize outcomes and ensure patient satisfaction. Moreover, implementing comprehensive preoperative assessments and postoperative care regimens that address hormonal balance, metabolic health, and nutritional status can further enhance the success of fat transfer procedures in older patients.
    We want to emphasize that the technical process of fat transfer—harvesting, processing, and reinjecting fat—is not the issue. Instead, the patient's overall health and wellness play a critical role in the success and maintenance of the results. This aims to raise awareness of how molecular hydrogen can reduce oxidative stress and chronic inflammation. Water quality, filtration, and health benefitsI met Dr. Paul Barattiero in September 2022 at a collaborative meeting where I was left impressed with his presentation of scientific literature and the development of his water filtration system. Dr. Paul’s journey began with his wife, who was ill for the first 10 years of their marriage, suffering from anemia, thyroid issues, and gynecological problems. In his commitment to help her, he discovered the benefits of hydrogen-enriched water, which significantly improved her health. This experience led him to create the Echo Water System, aiming to help others facing similar challenges.The sad reality is that water quality has drastically diminished over time. Tests and laws governing water quality are outdated, and the number of chemicals and pollutants in water has increased significantly since then. It's essential to drink pure water, as water with contaminants can be rejected by the body, leading to dehydration.Proper hydration with high-quality water is crucial for health as our bodies depend on water for optimal health, as it constitutes a significant portion of our blood, brain, and other tissues. Adding hydrogen gas to the water enhances its benefits by lowering inflammation and oxidative stress. Oxidation is a normal bodily process, but oxidative stress is harmful, and hydrogen helps manage this balance. By selectively targeting harmful reactive oxygen species, hydrogen reduces cellular damage and inflammation, promoting overall health.   • Although undergoing breast implant surgery has benefits, it also involves short-term and long-term risks like any other surgical procedure. There could be bleeding, infection, and scarring as short-term risks while for long-term risks, there are cases involving capsular contracture.
    Reasons as to why women get breast implants vary but more than that and the benefits of having implants, they should also be informed by their provider about the risks of breast implant surgery. That’s why I’m here to discuss breast implant surgery and the risks that come with it.
    What are some of the changes that can happen to breast implants as you age?
    We all know that as we age, we experience many physical changes. Some people lose weight and some gain weight. However, your implants will remain the same. But it is the breast appearance that we’re more concerned about as they may start to sag off of the implants.
    There may be a deformity as you age and your skin loses its elasticity over time. You will then have two issues – you’ll have the breast implant sitting higher and the breast tissue coming off of the face of the implant.
    When that happens, you’re more likely to need additional surgery. In case the tissue is aging off, you can recommend to your patient to undergo a lift surgery. This may be the case for women who were pregnant or had children and experienced expansion of the tissues with milk production and contraction after discontinuing breastfeeding. There may be stretch marks or other physical changes to the breast aesthetics.
    What is mastopexy augmentation?
    In relation to the changes caused by various factors, we mentioned having a breast lift surgery otherwise known as mastopexy augmentation. Its purpose is to lift, reshape, and tighten the breasts for the improvement of their appearance.
    If you're considering having procedures involving the skin, however, keep in mind there will likely be more complications related to skin infections. Obviously, there's more scarring when you're doing something like that because it's a visual scar that is going to be down the front of the breast and around the areola complex, unlike that in a primary augmentation, where it's probably under the fold around the nipple or in the axilla.
    Why you need to choose a board-certified or board-eligible plastic surgeon.
    Like I always say, it’s important for you to choose a reputable board-certified plastic surgeon for these types of procedures. They have undergone a certification process demonstrating their knowledge and skill to have them qualified to perform breast implant surgery.
    It's crucial to conduct thorough research and choose a breast implant surgeon who is skilled, qualified, and has a successful track record to ensure that you receive safe, high-quality care and get the greatest outcomes.
    As a professional who did oncologic reconstruction for a long time, I was faced with many challenges with people who have pre-existing medical conditions. These cases trained us to be careful at every step of the process.
    That’s why if a young woman came in for a consultation for a breast augmentation and upon collecting her information, I discovered she has a family history of lupus, rheumatoid arthritis, or other autoimmune disorder, I would not recommend that she get a breast implant for augmentation.
    Given their pre-existing autoimmune situation and considering that they would then progress to other therapies, hormone suppression, radiation, and others, their situations would be so complicated.
    Your surgeon should make sure that they’re doing their best to monitor your situation before, during, and even after your surgery to avoid any complications. Before surgery, a plastic surgeon should conduct a thorough evaluation of the patient's health history and family history, including any medical conditions, medications, and allergies.
    During the surgery, your plastic surgeon must take all the precautions in ensuring a safe and successful procedure. And after surgery, it's important for your surgeon to closely monitor your recovery and your overall healing process. Follow-up appointments and consultations are also crucial to address any concerns should there be any.

  • While breast implants or breast augmentation are generally considered safe and effective, there has been growing concern about their potential link to neurological symptoms. There have been several patients over the past years complaining about symptoms of neural inflammation such as brain fog, headache, sound sensitivity and light sensitivity.
    And that’s why we’re here to shed light on this topic. In this episode, we’ll answer the concern of many as regards breast implants and whether or not they can cause neurological symptoms.
    Can breast implants cause neurological symptoms?
    Whenever a patient tells me they’re experiencing headaches, light and sound sensitivity and brain fog, my initial thought is that it is related to neural inflammation and environmental toxicity.
    It must be determined whether or not the patient has been exposed to environmental toxins because these toxins would complicate neural inflammation. Many cases in Texas, Florida, California, and Georgie are rooted from this circumstance.
    We really have to take into consideration the environment the patient is in. Being exposed to moist environments like being near rivers, oceans, lakes, or the like where there may be high mold counts which you’re unaware of triggers inflammation and other issues or complications in having breast implants.
    If someone had a preexisting neurologic condition, this could certainly be worsened in conjunction with a mold exposure. And as we've discussed in our previous episodes, breast implants can be responsible for inflammation. Thus, these other disease processes can result in further inflammation and give you neurologic symptoms.
    Some research studies provide that immune reactions triggered by the breast implants, such as inflammation or an autoimmune response, may contribute to the development of neurological symptoms. Other factors like genetic predisposition, pre-existing medical conditions, and individual patient characteristics may also affect the development of neurological symptoms in some patients with breast implants.
    Identifying the Signs of Neural Inflammation
    We conduct a lot of tests to really identify the issues and further understand what’s happening. We have toxicity testing and inflammation testing which greatly helps in resolving these issues.
